Ohio requires children to be properly immunized for entrance into Kindergarten, 7th grade, and 12th grade. ALL seniors must have a recent meningitis immunization prior to the first day of school.
New immunization requirements for schools in Ohio will start with the 2016/2017 school year. Students entering 7th grade will need to have one dose of meningococcal (serogroups A, C, W and Y) vaccine in addition to the current requirement of having one dose of the Tdap (tetanus, diphtheria, and acellular pertussis) vaccine. Students entering 12th grade will need to have a second dose of meningococcal vaccine. The second dose must be administered on or after the 16th birthday. If the first dose of meningococcal vaccine was administered after the 16th birthday, a second dose is not required.
Vaccines are provided to children and adults at our walk-in clinics on a first-come, first-served basis at the Wayne County Health Department.

All children under 18 years of age must be accompanied by a parent, legal custodian, or an adult with a note allowing for care/immunizations. Proof of guardianship will be required of all legal custodians.
