Schedule Change Request Form

In need of a schedule change?  Please read & print the attached information regarding schedule changes.  This can then be sent to your guidance counselor or dropped off at the school. 

Counselors will be off for the summer beginning Friday, June 7 until Tuesday, August 6.  There will be no changes during that timeframe.
